DVA Patients & Coordinated Veterans’ Care (CVC)

Sippy Downs Family Clinic is proud to care for veterans and their families. We welcome Department of Veterans’ Affairs (DVA) Gold Card and White Card holders, bill DVA directly for eligible services, and offer the Coordinated Veterans’ Care (CVC) program at all ZH Doctors Network clinics.

DVA cards at our clinic

  • Gold Card – covers your health care needs for all conditions. GP consultations are billed directly to DVA with no out-of-pocket cost.
  • White Card – covers treatment for conditions DVA has accepted as related to your service, plus certain mental health and cancer conditions.
  • Orange Card – used at the pharmacy for medicines under the Repatriation Pharmaceutical Benefits Scheme.

Please bring your DVA card to every appointment.

What is the Coordinated Veterans’ Care (CVC) program?

CVC is a DVA program that helps veterans manage ongoing (chronic) health conditions, improve their quality of life and reduce the chance of unplanned hospital stays. Your GP works with a care coordinator, usually our practice nurse, to create a personalised care plan and keep in regular contact with you, in person and by phone.

Who is eligible for CVC?

  • Veteran Gold Card holders with a chronic health condition, or
  • White Card holders with a chronic mental health condition that DVA has accepted as related to service

You must also live in Australia, agree to a care plan and meet the criteria your GP assesses. People living permanently in residential aged care are not eligible.

How CVC works

  1. Book a GP appointment and let us know you are a DVA patient interested in CVC.
  2. Initial assessment – your GP and nurse meet with you in person to develop your care plan and goals.
  3. Ongoing coordination – our practice nurse checks in with you regularly and helps you with education, appointments and referrals.
  4. Regular reviews – your GP reviews your plan as your needs change.

Care coordination under CVC is free for eligible veterans.
