Chronic Disease Management
Chronic disease management at Sippy Downs Family Clinic is bulk billed for eligible patients with a valid Medicare card. Living with a long-term health condition such as diabetes, heart disease, asthma or arthritis can be challenging, but you don’t have to manage it alone. Our Sippy Downs GPs and practice nurses work with you to create a personalised care plan that helps you stay well, prevent complications and get the most out of life.
What is chronic disease management?
A chronic condition is a health condition that has been, or is likely to be, present for six months or longer. Under Medicare, your GP can prepare a GP Chronic Condition Management Plan (GPCCMP), which replaced GP Management Plans (GPMPs) and Team Care Arrangements (TCAs) from 1 July 2025. Your plan sets out your health goals, the treatment and support you need, and the steps you and your care team will take together.
Conditions we help you manage
- Type 1 and type 2 diabetes
- Heart disease, high blood pressure and high cholesterol
- Asthma and chronic obstructive pulmonary disease (COPD)
- Arthritis, osteoporosis and chronic pain
- Chronic kidney disease
- Obesity and weight-related conditions
- Thyroid conditions
- Long-term mental health conditions
- Stroke recovery and other long-term conditions
What your care includes
- A thorough health assessment with one of our practice nurses and your GP
- A written care plan tailored to your goals and lifestyle
- Regular reviews to track your progress and adjust your treatment
- Medication reviews and advice on managing your condition day to day
- Referrals to allied health professionals and coordination with your specialists
- Preventive care, including vaccinations and recommended health screening
Medicare-subsidised allied health services
With a GP Chronic Condition Management Plan, you may be eligible for up to five Medicare-rebated allied health services each calendar year. These can include physiotherapy, podiatry, dietetics, diabetes education, exercise physiology and more. Your GP will discuss which services suit your needs and arrange your referrals.
